JOB OPENINGS

Position: Help Network of the Mahoning Valley received a grant from Ohio Children’s Trust Fund to help support our communities through strengthening and preserving families. Cadence Care Network is the provider site in Mahoning and Trumbull Counties for this grant. This position will provide direct services to families including administration of screening tools, home visiting, parent education programs, resource management and referral, and family coaching.

Responsibilities: Specific activities will include, but are not limited to, the following:

  • Provide ongoing supportive consultations and case management services delivered either virtually or through a combination of phone calls, emails, skype, or in-person through individual meetings.
  • Provide short-term in-home services (i.e. one or two home visits each month)
  • Serve as a Family Coach to develop a prevention services plan, outlining the families’ goals and will utilize evidence-based programming to enhance the families’ protective factors and increase the family’s stability.
  • Provide parenting programming and facilitate access to Life Skills training, as well as Family Coaching, which will incorporate Motivational Interviewing to assist the family in moving towards behavioral change.
  • Attend meetings amongst collaborative members.
  • Collaborate with evaluation staff to facilitate and support the completion of the updated needs and resource assessment for the project and to adhere to evaluator guidance on evaluation/assessment administration.
  • Develop rapport with local social service partners and community resources to build and maintain a professional, responsive relationship with key community partners.
  • Responsible for ensuring data collected and documentation of services is consistent with rules and regulations, and requirements of program evaluation.
  • Submit regular programmatic and fiscal reports to the Program Consultant, as well as to other identified partners requesting data.
  • Actively engage with family and child serving systems, including attendance at community meetings, such as Family and Children First Council.
  • Serve as point of contact for Trumbull County.

Qualifications: The Family Coach position requires someone who is highly motivated with a strong background in working with families. The person should possess good written and verbal communication skills, strong organization skills, and the ability to work autonomously.

  • Preferred qualifications in this position are bachelor’s in social work or related field, or 5+ years of relevant experience, providing services to families or children, including prevention or treatment related to child abuse and neglect.
  • Has a strength-based, family-focused, and culturally aware approach.
  • Preferred qualifications include experience with evidence- based parenting programs.
  • Effective with computer software, proficient in Microsoft, experience with virtual platforms, such as Skype, Zoom, Microsoft Teams. Good verbal and writing skills.
  • Able to travel and work remotely Columbiana, Mahoning, or Trumbull County.
  • Demonstrated experience working within systems to remove barriers for families seeking services, as well as ability to arrange interagency meetings designed to coordinate services and ensure effective collaboration.

Cadence Care Network has an immediate opening for a part-time Behavior Support Coach for our Coffeehouse. The Behavior Support Coach will help Cadence Coffeehouse and Creperie Employees identify and modify behaviors that might be hindering their progression to other employment opportunities or career development by using therapeutic techniques to improve their work habits, communication style, and overall professional demeanor. The Coach will incorporate cognitive-behavioral approaches to address underlying thought patterns impacting their abilities to grow while providing support in and out of the coffeehouse.

Responsibilities:

· Provide behavioral and job coaching assistance to all staff members in the coffee shop.

· Coordinate and conduct activities with the staff that increase a staff member’s capacity to positively impact their environment with the goal of ongoing development of daily living skills.

· Help staff members develop the skills necessary to achieve independence by managing their anxiety, depression, or other externalized behaviors that are barriers to independence.

· Prepare staff for transition to other jobs and/or opportunities upon transition from employment.

· Provide education and training to the client, caregivers, and appropriate others specific to the client’s assessed needs and abilities. And provide ongoing assessment of staff members’ personal, family, educational, social and mental health needs for staff members.

· Coordination and assistance in crisis management and stabilization as needed.

· When a staff member is an active behavioral health client, provide mental health services as determined in the client’s individual service plan.

· Provide appropriate interventions that address symptoms, behaviors, and emotional well-being.

· When appropriate develop, complete, implement, and review staff member/client’s individual service plan.

· Develop understanding of coffeehouse manual and job functions to further support the staff.

· All other duties as required.

Requirements:

· Minimum of high school diploma with three years’ experience or bachelor’s degree in social work or closely related field preferred.

· Experience in mental health and working with youth and families.

· Ability to use independent judgement and make decisions governing the best course of treatment suited to individuals and families served.

· An understanding of family-centered services with a commitment to children’s safety and family stability.

· Ability to maintain a flexible work schedule that will meet the needs of the clients served, i.e, a non-traditional schedule that may include evenings and weekends.

Cadence Care Network has an immediate opening for a Behavioral Health Wellness Coach to work in a Trumbull County school system.

Responsibilities include, but are not limited to:

· Collaborate with school leadership to coordinator, plan, implement, evaluate, sustain, and innovate the school district’s overall Student Assistance Program and Staff Wellness programming and practices. Build and sustain awareness of this programming with students, parent/caregivers, and the community.

· Work with building administrators, school counselors, and other student wellness staff to connect and coordinate behavioral health services and staff wellness programming.

· Work collaboratively and coordinating the development of the school-specific Student Assistance Programs (SAP) forms, procedures, and policies with the Associate Superintendent of Student Services and other school administrators.

· Provide coordination and support of promotion services for behavioral health/substance use, early intervention selective services and supports, indicated services and support or behavioral health problems (including substance use).

· Provide staff development and program awareness to school staff related to SAP, staff wellness program and other related topics. Including establishing and coordinating SAP teams and existing staff wellness efforts to implement a staff wellness program in each of the school buildings.

· Identify methods and measures for a clear evaluation plan of SAP and staff wellness programming, including implementation of evaluations/surveys/needs assessments both within the school and with community partners; collect and maintain data and compile evaluation reports.

· Identify, network, and enhance community partnerships and resources through participation in community committees, coalitions, councils, community events, which may include before/after school hours, weekends, and during scheduled school breaks. Work with community partners to provide prevention education, intervention, and treatment services within the school buildings.

· Provide limited prevention education programing within the schools and/or community as identified and approved by supervisor(s).

· Participate in professional development including school professional development days as identified by supervisor and District Leadership Team.

Qualifications:

· Bachelor’s degree preferred; associate degree required.

· Must be an Ohio Certified Prevention Specialist, or able to show evidence of working towards the Prevention Credential including Registered Applicant (RA) status.

· Experience of working with local school districts, law enforcement, and /or human services agencies.

  • Experience with, or knowledge of, evidence-based mental health and substance abuse prevention strategies

The ideal candidate for this position possesses the desire and ability to work with youth/students within the school and community setting to identify youth:

Cadence Care Network, a local non-profit foster care and behavioral health agency, has an immediate opening for a Care Coordination Supervisor.

Responsibilities include:

  • Participate in the recruitment, interviewing, and training of potential new Care Coordinators
  • Formal and informal ongoing training of assigned staff.
  • Ensure achievement of monthly and yearly goals for staff and program.
  • Conduct evaluations and develop growth plans for assigned staff for continued professional development.
  • Provide weekly supervision with assigned staff, including facilitating weekly staff meetings.
  • Oversee the development and implementation of all Plans of Care for clients and their families.

Cadence Care Network has an immediate opening for a CME Care Coordination Program Director for Mahoning and Trumbull Counties.

Responsibilities include but are not limited to:

· Maintaining adequate staffing levels through recruiting, onboarding, and training of full and/or part time staff

· Managing the financial performance of the applicable services through collaboration with the finance department.

· Develop and manage utilization requirements and quality outcomes for the program with compliance and quality improvement department.

· Ensure that all CME services operate at maximum potential to meet community needs in a cost-effective manner.

· Participation in the development and review of appropriate financial and service outcome metrics.

· Ensure that services meet the expectations of the contract, including both but not limited to, providing required reports, financial and staffing management, achievement of monthly and yearly goals for program, in addition to completion of GKR and monthly radar reports.

  • Assist with new employee onboarding as it relates to review of training, monitoring and tracking completion.
  • Enter and track ongoing employee training. Schedule training as needed.
  • Perform monthly audit of employee files to track for expiring documents, i.e. Driver License, Auto Insurance and Certifications, etc.
  • Data entry and filing of HR related documents.
  • Assist with pre-employment activities and onboarding as needed.
  • Serve as a backup for the payroll function.

This is a great position for college students majoring in psychology, social work, and other similar programs.

Responsibilities include but are not limited to:

  • Mentor young people and model skills that will help them successfully minimize the negative impact of behavioral health issues.
  • Engage clients through youth-friendly, culturally appropriate, and youth-empowerment approaches.
  • Participate in the implementation of tailored supports and services to be individualized, developmentally appropriate, appealing, non-stigmatizing, trauma-informed, and relevant to each young person.
  • Teach and reinforce social, daily-living, and community functioning skills as relevant to a young person through role play skill development and practice.
  • Assist in Prevention Planning to minimize a young person’s engagement in high-risk behaviors or situations – and perform crisis interventions as situations arise.

Cadence Care Network, a local non-profit mental and behavioral health agency, has immediate openings for Care Coordinators.

  • Care Coordinators are responsible to provide wraparound care coordination for children and youth, and their families, who have the most complex behavioral health needs.
  • The Care Coordinator will develop and maintain the wraparound team.
  • Coordinate, and supervise implementation of the Plan of Care, including a Transition Plan and Crisis Plan through service delivery with providers and community resources.
  • Ensure family support and stabilization during crisis; and maintain all documentation and reporting as required.

Candidates must have experience in one or more of the following systems:

  • children’s behavioral health, child welfare, developmental disabilities, juvenile justice, or a related sector human service.
  • Or behavioral health care field, providing community-based services to children and youth, and their family or caregivers.
